1– Submit the online information form

You complete the online questions in the Information We Need section. Click on Send at the bottom to request the rates. You will get an email right away to let you know if you qualify for a premium discount or not, based on your answers.

2 – Submit a Request for Conversion of group life insurance form

Your employer needs to fill out a Request for Conversion of group life insurance form either online or in paper format.

The online version is easy to complete and is automatically submitted directly to us. The paper format can be returned to us by fax or by email

When can you send it?

We need this form 31 days before or after your termination date to keep your right to convert.

  • Don’t send it in too early – it can’t be processed
  • Don’t send it in late – more than 31 days after your termination date, your right to convert has expired

2 – Getting a quote

Once we get your form, we confirm your coverage amount, termination date, and date of your new coverage. We will send you an email or letter with your available choices. It can take a few weeks before you get the information from us.

The cost of your new coverage depends on your age, amount and type of coverage, and if you have a discount. You can choose from One-year Convertible Term, Level Term to age 65, and Whole Life. You can even combine them.

For example, if you have $100,000 to convert, you could take $50,000 of Term to age 65 and $50,000 Whole Life. This gives you coverage of $100,000 to age 65 (when the level term ends), and $50,000 for life after age 65.
